Before playing in an AFDC League you must acknowledge the following waiver. If you are a minor you must also print this waiver, sign it, have your parent or guardian sign it, and mail a copy to the address at the bottom of this page.

I ACKNOWLEDGE, UNDERSTAND AND DECLARE THAT: 


1. I am in good physical condition and, to my knowledge, have no disease or injury that would be aggravated by participating in Fall League Ultimate Frisbee or Goaltimate organized by the Atlanta Flying Disc Club, Inc. (the “AFDC”).

2. Participating in AFDC Summer League or assisting others in participating may involve the RISK OF INJURY to me, including death, loss of property or damage to me or my property, or other consequences which may result from my own actions, inactions or negligence, and by the actions, inactions or negligence of others, the rules of play, or the conditions of the premises and any equipment used. There may be other risks not known or not reasonably foreseeable

3. UNDERSTANDING ALL OF THE ABOVE, I HEREBY ASSUME ALL RISKS, KNOWN OR UNKNOWN, related to my participation in AFDC Summer League, and in connection with my entry in, my participation in, and my performance or lack of performance in any and all Summer League games and events, I hereby release, waive, discharge, hold harmless, indemnify and covenant not to sue: the owners or operators of any and all facilities used in Summer League games and events; the AFDC and any subsidiary, successor, affiliate or related company or business; the members of the AFDC, including its officers and Steering Committee members; any players, captains or spectators in or at Summer League events or games; any of the AFDC’s agents, representatives or volunteers; and any unaffiliated organization participating in any way in AFDC Summer League play or events. THE FOREGOING RELEASE, WAIVER, DISCHARGE AND COVENANT EXTEND, WITHOUT LIMITATION, TO ALL LIABILITY FOR INJURY, INCLUDING DEATH, LOSS OR DAMAGE TO PERSON OR PROPERTY, AND ANY OTHER CONSEQUENCE THAT MAY RESULT OR ARISE FROM MY SUMMER  LEAGUE PARTICIPATION.
